Self Referral: -
Please note, although we do accept self referrals, we strongly recommend that individuals gain access through a referring agency. Individuals are advised to contact a key worker.
Referring Procedure stage 1 and 2: -
Stage 1: -
All referring agencies are required to download and complete the following forms on behalf of the client: -
1. Download a copy of REFERRAL FORM
2. Referring agent download a risk assessment form, complete it and return with the referral form RISK ASSESSMENT
Stage 2
Statutory Check for Referrals:
Referring agents are asked to provide the following information. Failure to provide the following information will delay the referral process.
1 Completed Referral Form
2 Risk Assessment - download the above risk assessment - OR - provide an alternative risk assessment.
3 Provide a brief covering letter generally outlining the referral case.
Please note that applicants are not guaranteed a placement. Placements are allocated subject to assessment only and strictly on a location priority basis.
